The Q Clearance is the enigmatically-named security clearance created to allow access Restricted Data, the legal category for nuclear secrets in the United States (after which my book is named). It is issued by the the US Department of Energy, and requires a single-scope background investigation (originally by the FBI), with the same requirements as a Top Secret clearance, and keeping it requires being re-investigated every 5 years.
I was just pointing that the requirements to get Q clearance are the same as to get Top Secret clearance which a lot of people have. The only difference is you have to work in the Department of Energy and deal with restricted data which is like classified data.
Even if you have the top level security clearance at the CIA, you don't have Q clearance, but the scrutiny to qualify for either is the same. It's the same type of background check, it just depends what your job is. Q is different from TS, but not higher.
