I am certain that while Trump was covered, several agents did a full sweep of his body to confirm if there was any other wounds, apply tourniquet(s) if needed. His shoes would have been knocked off from their sweeps. It is also why his shirt was disheveled when he was raised up(like a baptism), because they would have opened up to make sure nothing got through the vest.
