I received a voicemail yesterday of a prerecorded call by the CDC asking me to call back to take their National Immunization Survey. I called back today hoping to give them a big fat "fuck you", but was disappointed to find out that you just push buttons (which I wasn't going to do). When you hit the option to leave a message, it hangs up on you (I think they know what they're going to hear).
Anyway, I did some quick searching and found that this survey is run by the University of Chicago.

Probably need to look into this a little more; they'll even send a survey to your child's doctor "with permission" to confirm the validity of your responses.
