Let's say i work for a governmental entity, and we are getting pressed to complete this online vaccination survey before we return.

They say it will dictate whether we have to wear masks all the time, and they will have our managers follow up with us monthly.

They only require that we attest to how many shots we took, and what was the date if the last injection.

Seems awfully tempting just to fill that line out... but I am not sure if that will blow up in my face or what the implications of doing that would be.

Has anyone dealt with anything similar?
