I've seen it several times with mental health/dementia patients.
It's like they have a lapse in memory of where they are or why they're there and instinctively become mildly angry/agitated for a second before they snap back to and get their bearings.
That is legit the weirdest expression ive seen in my entire life. His eyes were empty for a while.
