I copied a form that would suffice for a religious exemption for vaccines in school children in Virginia then had it notarized online with a notary public. I'm pretty sure it could be recreated using any word program in less than an hour. It cost me less than $40 and I'm the only person on the job that doesn't have to wear mask.
