Any number of causes, it can be overloading and improper fusing or relaying, it can be icing causing shorts and flashovers, it can be the weight of ice making it sag and break lines from the wind.

Ice and freezing weather causes power plants to have troubles too so as plants do shut down loads increase on operating lines.

A lot of things cause it, not to mention hackers and Enemies.
