We found out yesterday that he has pneumonia. Once I found that out I went and got him from that ER. Today we find out he’s negative for covid. Am I overreacting for wanting to contact administrators for possibly causing harm to my father? He had BACTERIAL pneumonia and they forced 2 masks on him while short of breath. I’m more livid than before knowing he doesn’t have Covid.

I think it is generally a good strategy -- keep records and make "contemporaneous" notes about those records.
In this case, it would be the father filing the lawsuit (if he chooses). If the son or anyone else keeps the records and notes, it could probably be entered into evidence (with the court clerk), but should (must?) be filed with an affidavit -- similar to what you propose, but more "legal."
And then the maker of those records would be a witness, and subject to cross-examination, but it would be important evidence for a jury.
Has to be done by the Rules of Evidence, though.
