That's either a battery pack for a microphone or it's an ankle monitor. Those are the only two things that will make that bulge.

Knowing Oprah and her penchant for ALWAYS looking her best, it makes no sense to put a battery pack on her leg. Not with the length of wires needed.

Which is why, even subconsciously l, we KNOW this is an ankle monitor.

Occam's Razor applies here.
