Back in my youth, and we're only talking the 80s and early 90s, masculine girls were called tomboys and effeminate boys were called sissies. The latter is admittedly rather pejorative, and could lead to bullying/esteem issues, but beyond that, these individuals either grew out of it, or grew into homosexuality, which as adults was by then well enough accepted.
