Technically the "wars" are all DS military interventions to secure resources or provoke destabilization that leads to black ops that profit them (human and drug trafficking, smuggling of general goods, etc). Last real war we've been ranges from Korea to the Civil War, depending on your perspective.

Biden was threatening Russia basically completely disrespecting them. Russia and China both view US as historically weak right now since Biden is in office. If Biden decides to escalate tensions more in Ukraine could be a powder keg
DEFCON is a "state of alertness", so to speak. It's a general indicator that guides military protocols. It goes from 5 (SSDS, like Call of Duty said. Small localized skirmishes.
