It's just a gender swap app. They have it on snapchat. It's 1,2,3 and you can use a filter to swap the gender. I've had friends who tried it out. It gave them a beard and jaw. To the men it gave long locks and feminine features. And to the soy boys it showed them what they'd look like if they had testosterone.
The above photo is Bill and his (soon to be ex-wife) Melinda. The bottom one is just when someone used a gender swap filter on their photos.
0 sources = *hearsay. A snapchat filter above a picture would not produce a clean image like this. It possibly computer generated but idk why that simple explanation is not given anywhere I looked. I have spent many hours looking into this, you have spent around one minute typing *hearsay and assumptions. That is not valid. I specifically asked for the source or meaning of this image, not an opinion.
Literally the default FaceApp options. Took 2 minutes. Someone could easily take a few more to clean it up and change facial features, hair options, etc
The answers easy. To make the meme someone gender swapped the bottom and then did a little photo shopping. Of course the bottom one has been tampered with.

I don't know. Melinda today looks waaay more like a man than she used to. Looks like a different person. Weird.
nope. That is Kevin Kline playing the part of Melinda.
I considered this but can you do that with these images and demonstrate?
Just get the snapchat filter or watch a you tube video see how the filter is used.
It's ridiculously easy to do.

The photo you posted has an obvious crop on the long hair.
