Because this is a massive experiment. There are placebos in place, some get it some don't, that's how you can have a ship full of vaccinated people, with several getting covid. That's also why every clinic/hospital has to strictly track the lot numbers of the doses given. Because it's double-blind, that would be the only way to know what is saline and what is not.
