So who was the 4th family? Who are all 4 families? house of Saud, UK Monarchy? Rothschild?. I’ve never seen a straightforward answer...

Nobody knows for sure. Later Q posts confirmed the 4th (removed) family as the House of Saud. One of the three families may be Rothschild (Q talks about them quite a lot). UK Monarchy may be a good idea from you. I'm optimistic that we learn more about this topic later.
