there is little data entry standardization (i.e., too many fields allow the entry of non-standard [freeform] data and text), so filtering the data for analysis is a pain;
a lot of entries get thrown into "other"-type categories which effectively buries the info like needles in haystacks; and
MAYBE WORST OF ALL, INITAL ADVERSE REACTION ENTRIES DON'T GET UPDATED. So, an entry for a rash reaction is not updated to a death if that person dies after the initial report is entered, thereby artificially LOWERING THE DEATH STATS BY DESIGN.
