So, a friend used my phone to do this recently. Called the location where got the vx, said she would like to ck status for an employee. The pharmacy employee said “I think I can do that, what is the date of birth” then hung up. Seems can call the place where the card states it was given and verify from there.
There is no federal database for cards. There are some state databases (CAIR is California immunization registers) in California and some blue states.
I'd suggest replacing that card you got sometime around April at one of those drive up events they did at stadiums when you were in a more red state like AZ when you were there on vacation.
Those events were staffed by day laborers and volunteers so there was no medical chain of custody for HIPPA records.
Then you can take that card back to your PCP / Dr in one of those yucky states and ask if they can add it to your file.
Don't make up batch numbers if you can't remember what yours was. Either copy one from the same state you "got" your shot or leave it blank. They are somewhat state specific.
I bet you they'll make you get revaxxed for a new one.
I ended up with an extra MMR vaccine because the government in Taiwan said it's required for a work permit here, and I only had like one day to prove that I'd already taken it, so they just told me to get a new shot right there. I didn't really think that much about it at the time because I had a ton of different things to quickly deal with at that time and didn't need the complication but now I wish I'd fought that.
Right now they're turning the screws hard on the vax here. This is probably what makes me return to the US... Except my loved ones here will likely not go with me.
Well, seeing as the jerk woman who was interviewing the boyfriend said she was uncomfortable being in the same room with an unvaxxed person, I'd say he wasn't going to get hired.
But for future reference and slightly different circumstances you have a great idea! ;-)

Ban the unvaxxed from society
Write articles implying the dirty unvaxxed are making fake cards to sneak into society
Justify having a digital "vax passport"
It's gonna get way worse.
