A one question survey: "Are you vaccinated?" There's supposed to be 3 choices 1) Yes 2) No 3) Decline to answer
Spouse will be selecting option 3. Not too sure what type of company survey wants your name except for a one that's looking for trouble.

Employers can legally ask but employees are not required to answer. That does not, however, stop employers from "releasing" employees who refuse to answer.
