There’s 2 different ways to say “no” on her questionnaire at her work (won’t disclose exactly what but this job is very close to the medical field). One is “for deeply held religious beliefs” and the other is “I’m not getting vaccinated for reasons other than medical or religious beliefs” Sounds like being honest might be admission of guilt to the max, basically outing herself. What would you do?

I would go with the deeply held religious beliefs. Religion is a very private matter. She doesn't need to attend a church or evangelize order to hold her beliefs. And if they try to push it she can claim religious discrimination. So I think that's the safest route to take. It sucks that she has been put in this situation but sadly she's not alone.
