Formulary restriction: it means the hospital is telling the doctor what they can and can't prescribe for all their hospital patients. Kind of like how each fast food place is either Pepsi or Coke, the hospital admins limit what each doctor franchise has access to.
Except it's more complicated and legalistic, and they pretend it's because it helps someone besides their pocketbook.
