NEED HELP WITH DRAFTING/GETTING EXEMPTION TO CLOT SHOT

Frens, I am reaching out to this vast network of individuals smarter than I for help in drafting/gaining an exemption to mandated clot shots by an employer. I am aware of the resources that have been compiled here, but I was wondering if any of you have applied for and were granted an exemption. I am employed by a county level governmental agency who has mandated the clot shot by Dec 1. Any help with this matter would be greatly appreciated!
Type of a form that lists all of the known adverse effects, and the fact that the shot doesn't prevent contraction or transmittion of covid19. Make sure there is a line that confirms that the vaccine manufacturers are free of any liability in the event of any adverse effect. Make it so that they have to innitial each line to indicate they read it. Then at the end state that since the employer is mandating the vaccine the employer accepts full financial responsibility in the event you suffer from any adverse effects, or contract covid19. Make them sign it. They won't. If they fire you after this, threaten to sue them over medical discrimination. Talk with a lawyer first if you want and have them draft the letter and send it on your behalf for more impact, or just do it yourself.

I have a relative that is in a similar situation. They are going to be reached out to individually in the near future and have to state their vaccine status. Deadline to be fully vaccinated in Dec 8.
The contractor company has stated that they are leaving politics out of their decision to follow the mandate which they stated is federal law. We know this is untrue.
Take a look at the link below. Since the employer has stated that they are leaving politics out of it, it may be best to approach this exemption solely on religious beliefs. I am trying to help this family member out so please share which direction you do decide to run with and keep us updated. I surely will do the same on my end.
