Note that "reason of personal conscience" is limited to religious objection. I would prefer that it say "personal objection for any reason".

I think you're misreading it. It's a comma with an or listing out three independent reasons:

ANY REASON of Personal Conscience
Religious
Medical (including Natural immunity)
