Nor is it an an actual ADM. Not even the same league as the Surgeon General, which is a fictitious "military" title with an even more fictitious uniform.

Well, the USPHS is an actual uniformed service, which developed out of the navy basically to enforce quarantine at ports. And to this day they keep up the ranks and stuff. Otherwise they are play sailors, less than the ones in NOAA, for instance.
