An email went out to all employees last week that they hired a third party surveyor to collect vaccine status on everyone. The language basically said that although we haven’t implemented the mandate, they would need to know if we had it along with proof, or if we had not received the jab, supposedly for statistical purposes. We are required to respond no later than 11/22. Everyday since, I have received a reminder that I haven’t responded and that I’m required to by the deadline. I thought that surveys were optional(?). What happens if I don’t?

I don't know if this will help your situation but here's how it got handled in a hospital situation over the last week.
So, I go to the hosptial for chest pains and a blood pressure of 208/112. It happens from time to time.
While there, I was forced to take the "quick" test. Got the results back in 20 minutes. Negative. But 20 minutes, sitting in the waiting area because there's no entering the emergency room without a current test.
So, I get into the emergency room and there's a constant stream of nurses and nurses' aids asking me about my vax status. Ain't had it. Ain't getting it.
No real effort to treat my health issue. Some futzing around. But no effort to bring the bp under control or even reduce it. But I do get the usual horror stories about "all the unvaxed people we have dying up on the ward!" from all of them, constantly, in rotation.
Don't have it. Ain't getting it.
Finally, I call the "doctor" over. Mentioned, calmly: Nuremberg Code. Crimes against humanity. Zero proof the vax does anything beneficial. Overwhelming proof vax is killing and injuring many many people. You are a professional. Your duty is to keep up with info as it develops. First do no harm. No where in your oath is support The Narrative. Everyone in your industry will be held accountable. This is without doubt. Pushing the vax at me is samey same as advocating I submit to assault with a deadly weapon.
Well.
No more vax horror stories. No more constant rounds of nurses and aids asking about my vax status.
Didn't even have to wear a mask anymore.
Got treatment.
4 days later, I'm out of the hospital and doing fine.
