Refer them to the ruling by the 5th Circuit and the fact OSHA has SUSPENDED moving forward on this BS
I'd tell them to get back to me when the OSHA "Temp Standard" is ruled legal by the Federal court system. For now it's been stayed because it appears that OSHA is operating outside their authority. Until OSHA is deemed to have the proper authority to mandate the collection of private medical health information, which is currently illegal under HIPAA law, you will not be complying with this mandate. Document and save all correspondence in the event of a future lawsuit.
You have many options here.
I have no advice for you because I don't know your situation.
If my employer demanded this I would ignore it and if they insisted I would refuse and let them fire me. But admittedly I wouldn't suffer much hardship to be fired.
The more people that refuse to comply the better. But family comes first IMHO.
There are other jobs
I received something similar at my job, I ignored it and when asked in person about my status I say "my medical information is private".

All hospital workers, clinics, doctors office personnel need to wal out and cripple the entire health care system. Demand a letter that says they will be legally responsible for vaccine injuries from an EUA shot that provides 2 months of protection and a lifetime of misery with side effects. Remind hospitals of Nuremburg I.
Submit your vaccination status and start preparing a religion accommodation request. Keep it religious and personal.
