You only need to enter last name of the doctor, city and state. You can leave the rest of the search form blank

I didn't find it. It found me! I was reading through some medicare information my father received and stumbled on it! His primary care doc retired and we needed to find another. You can believe we didn't pick the one who gets avg 260k payouts each year. Another doc in the same practice averages 180k. The group certainly figured out their side gig!
