I have this same chart as a pic on my phone. I send it to people in my circle frequently.

Was sent you me somewhere along the line by my brother-in-law... created the post to answer another post asking for dosing bc I couldn't figure out how to add the photo on his thread. Second ever post and got stickied... so I'm doing damage control bc the post itself lacked info I am learning.
