The hospitals will force this on doctors that are on staff. A private practice doctor that takes hospital call for emergencies will likely just resign from their staff privileges at the hospital and simply work in their office. Examples are opthalmalogists, ENT doctors, Oral/maxillofacial surgeons/Plastic surgeons, etc.
They will not be able to staff the emergency department with these specialties. Many of these doctors make more money in their office than they do getting middle of the night calls to go to the hospital to treat a trauma injury or urgent illness or infection. When hospitals cannot supply the expertise needed to treat such things they will lose their accreditation.
Ex: Oral/Max surgeons will sit in their office pulling wisdom teeth, doing dental implants, bone grafting, and simple pathology rather than going in at 2AM to treat a jaw fracture for which they likely will get very little reimbursement if any. Eye doctor won't go in to treat a stick on someones eye. ENT doctor won't go in to treat a deep neck infection or GSW to the neck.
They have really fucked themselves as to what is coming. Most of all they have fucked the people that depend on those services.
