Malicious and malignant is a bit of a projection/stretch. Is it possible? Sure. But what is most likely - and most importantly, easiest to sell to the brainwashed masses - is that he’s simply incompetent and greedy. Personally that’s the angle I approach when he is brought up and it seems to work well.

Selling the “evil” stereotype is just too difficult because it requires someone to break down what they feel about doctors “ooo look at the white coat and title and degree - clearly they can’t be bad guys ever”.

Sell them on what is easiest and they will eventually work their own way to figuring out that “hey wait a minute … maybe this guy isn’t just greedy, he’s a fuckin tumor we need to cut out!”
