I agree there were nursing errors. The manufacturer did not make an error. The incorrect reagent was used to reconstitute the vaccine medication. The company would have liability, I would think. Without being able to track the patients who received the incorrect medication, we'll never know.

Maybe I'm not understanding you clearly. Which drug manufacturer should have been informed?

If you view the video, they were informed and gave the information that they had not reconstituted the vaccine with bacteriostatic water and therefore could not give any information on the result of that combination.
