I’ve had some and I’ve also had plenty who weren’t. It’s getting to be pretty easy to tell now what ones are good and really care but they are few and far between

Obamacare actually provides for 'independent panels' to adjudicate when these outlier cases are presented and to determine coverage direction and curb insurer/Medicare spending.
