I NEVER put down my race /ethnicity on any form. I always choose "Other".

I NEVER put down my sex on any form. Even if the form doesn't have this option, I create a box and mark "Other".

If a form asks my age, I always write in "Varies".

You never know what someone will do with the info... so don't give it to them.
