This seems unique. I've been employed under several employer health plans, as has my wife, and neither of us have ever had to answer so much as a questionnaire let alone perform a test.
The huge company I take money from offers cash/discounts for a physical, I think. I just skip it, I keep my data and they keep there carrot.

Yes, some sort of health baselining and to get you seen by a dr.
Extra life insurance requires a different exam. Term life separate from my company is cheaper, and it’s mine whether I work here or not.
My company does that too but I’ve never done it either.
