The cognitive dissonance is strong with these people.

We are told that there are not just two genders. There are at least 150.
Then we find out that people need operations to assign one of the two genders to them.
Also, they need either male or female hormones to complete the task.

It is like the T-shirt ads with pictures of rainbows extolling the virtues of the 150 genders then asking if the fit required is M or F.
