I lost our pediatrician of 13 years when we refused Gardasil for my son. Not that he refused to treat him, but we were there for his last well child check up and he was almost like a family member, had been there for us through many important health moments over the years. But this was the hill he chose to die on, and became a complete ass, disrespecting me to my son in my presence! Politely but firmly, my son slid off the table, said, "I'm not comfortable with this" and went out to sit in the car. I reamed the doctor. We had never turned down a vaccine but this was not required for school and I had heard too much scary stuff about it. Still upset when I think of that situation and it's been 9 years.
