It appears to be a large bruise. The elderly have thin, frail skin. A simple knock against a wall or a to vigorous grasp of the hand in a handshake could cause that. Also if blood were drawn in a hand vein you might get a sizable bruise or if an IV was given in the hands to administer blood, iron, hydration, antibiotics.
