However, per the link, exceptions can be granted under certain circumstances. They require the traveler to sign an attestation saying they meet one of these exceptions, but it is intentionally vague on whether additional proof of that exception is required. The link for the full list of requirements currently seems to be broken. They also want you to test when you arrive and self-quarantine, but with the availability of at-home kits and no more contact tracing, I don't think there's a way to enforce it.
I would check with your preferred airline to find out what documentation you would need in addition to signing the attestation. Another good source for documents and requirements is your local passport/visa agency. If they are available where you are, I highly recommend CIBT.
If in the end the only requirement is signing the attestation, I am of the opinion that "the shot will kill me" is a valid medical reason, but I am not encouraging or endorsing lying on an attestation form. You should do what is best and appropriate for you and your circumstances, and I hope this information helps you find your way here.
