I've started looking around for a new vet - I'd like to give them a chance before I just up and leave but like OP, want to go about it the right way and haven't been able to come up with one.
How much of a chance do you need to give them? We're well past the time for that.
It I was you, I'd just find another vet and go there, and I wouldn't feel obligated to say a word to the original vet. Losing clients is likely nothing new to them, and they might not even care what you have to say.
