I don't think this theory is correct. Often the inclusions seen in videos or images purported to be from the vaccine are straight not curved organically like a worm would. Further, the scale is totally wrong. The inclusions are orders of magnitude smaller than these worms which are visible to the naked eye.
The FDA's CGMP required of any facility that makes these explicitly maintains a sterile environment in the solution that precludes these worms from being there. If there's a contaminant, it is inorganic IMO.
Edit: Occam's Razor says look for the simplest solution as it's most likely to be the correct one. The simple answer is that the spike protein itself causes harm. It interacts with the ACE2 receptor interfering with the conversion of angiotensin II (a pro-oxidant) to 1,7-angiotension (an anti-oxidant). The concept is that the spike protein interferes with this effect, creating a localized region of oxidative stress in the blood. This results in microclotting and may be the cause of the myocarditis we hear about if it gets to be particularly severe. In most people, normal levels of antioxidants simply manage it and the angiotensin II is broken down by other means, but in high performance athletes working very very hard, they create a temporary oxidative stress. The effects combine with deadly results. Now, to be 100%, that's a theory, and both the CDC and FDA would deny there's any risk whatsoever. But, I believe it to be accurate, and why we see the results having selectivity for people who are already under high levels of oxidative stress from other conditions: infections, cancer, various metabolic diseases, extreme exercise, etc.
TLDR: I think the spike protein itself is the problem, not some random inclusion
