Likely from clotting of small blood vessels, either supplying the hearing portion of the inner ear (cochlea) or the nerve for hearing itself (auditory nerve). The Death Shot isn't called the Clot Shot for nothing,

We just learned a good friend of ours suddenly lost hearing in one ear (he's in his late 60s). Unfortunately he's a musician....
