I find it suspicious that a plastic surgeon added vaccination to his practice.

Possibly , but I knew people looking for ANY dr who was favorable to our side to get proof of jab, ivm and/ or hcq. MD is MD, not limited by specialty designation. He saw an opportunity to fight a battle with the tools at his disposal.
