Because Warren Buffet and these companies were able to successfully lobby against adding adequate braking systems to supply trains due to their business interest in shipping oil and hazardous chemicals via rail.
IMO, it was only a matter of time before a terrorist attack like this happened by rail. There’s hardly any vetting that takes place for engineers, and our rail infrastructure is in incredibly poor condition.
