They don't. Most of those are collisions, very minor. Also, anytime a wheel gets off kilter, it's technically a derail when all it takes is minor repair. Nothing like having an axle on fire on a hazardous goods train for thirty miles and administration overrides the standard safety procedures until it melts off.
