Actually you can have a massive and rapid change in weight and muscle mass with the right diet, exercise and nutritionist and doctor. Look at Christian Bale in The Machinist to Batman. He looked like he was going to die of starvation in The Machinist, then got jacked for Batman.
Other way around is WAY more difficult and if you have never seen someones bony frame the impression upon seeing it for the first time can be quite different.
However when you already know someones build being slender and slight you know what kind of frame the new mass/muscle is hanging on and the current 'Elon' isn't built like a slender frame with muscles on top...
