Apparently: (a) He succumbed to his mental disease, (b) He'd deluded himself into not only thinking that he can actually be a woman, but that it was reasonable to expect his wife to pretend along with him, (c) His utter selfishness in elevating his twisted, deranged fantasy above the responsibilities of being a good husband to his wife and father to their children is what actually "came out", (d) His mental illness has allowed him to fantasize that he was not actually born as male, but was "assigned" as such, (e) In his deranged mental state he could not foresee that his wife would not want a perverse influence around their children and that she would take steps to protect those children.
