My understanding is that these decisions directly impact the whole area in the court's district, but that other courts can reference the ruling in their next pertinent case. This means it isn't immediately in effect for the other districts, but likely to be as soon as a similar case comes up. (Edit- only nationwide immediately if it had been the SCOTUS)
