The Iron Dome didn't fail. There's tons of footage showing it working all over Israel. It was overwhelmed by sheer numbers. Cloward-Piven, if you will.
The wall also didn't fail. It stopped most people. It slowed a determined attacker and forced him to go to lengths to breach it. They used hang gliders, boats, and bulldozers, but they couldn't just drive through in minivans. The wall did provide value.
I strongly doubt the cameras failed. I suspect they worked, but the response caught IDF unawares (it was Saturday, the Jewish sabbath, and a day after a high holiday). It's one thing to have the information. It's another to be able to act on it. Rules of engagement may also have been an issue until it was clear this was a determined attack and higher-ups gave orders to go to war ROE rather than security ROE.
I also strongly doubt Mossad failed. This attack was highly coordinated with thousands of men, multiple spearheads of attack, etc. They struck fast and hard and in a way that had to require basic infantry training as well as specific practice for this particular assault. You don't pull that off without a big logistics operation and you don't pull off a big logistics operation without Mossad or CIA or other such agencies knowing about. Whether they assessed the threat properly and alerted politicians who can actually act on that intelligence is a bigger question. Naturally, since the politicians didn't act, they're going to pretend to be stupid and pretend they had no idea it was in the works when in reality they ignored intel because they were more focused on other issues, like abusing little kids and trampling the civil rights of political enemies.

And there were definitely NO IDF among the ranks of the attackers helping to coordinate and implement the attack like the FBI on J-6.
