Emphasis on heavily! Or a body double. The texture and tone of the skin, even the arms and neck, is too smooth and lacks age spots, sun damage, etc.

Botox works on wrinkles not age spots/sun damage, loss of elastin. Look at the link provided above by CasuallyObservant and compare with this image. This image looks more like a 30-something wearing a rubber mask. IMO
