Maybe the "vax" didn't travel well? It was supposed to be kept at a very low temperature - way lower than a basic 'fridge can manage - so maybe long distance batches deteriorated. And maybe some countries trained their needle wielders to "aspirate" before pressing the plunger.
I think that India suffered quite badly, too.
