Second ER visit in 2 weeks, this time with my ill husband who had fallen and suffered bad cut to head. I was astounded by the incompetence in the ER. No doctors, just a nurse practitioner there this time.

There is most definitely an uptick in incompetence in ER! After the third trip to an ER finally a diagnosis of sepsis and some imaging to find the root cause for someone I took to the hospital. The third trip I chose a different hospital and finally got some service. Try a different hospital, bigger, more reputable. Drive further to get there if you have to. Follow your gut and ask for additional tests/imaging if necessary.
i did the opposite. i went to an ER closest to me, sat there for five hours in complete pain. test were ran, no pain meds given. they told me they only found a little infection, dr said "big woo" (her words exactly) then said she wanted me to stay and see the surgeon in the a.m.
i said, if you don't know what is wrong why do i need to see a surgeon??? she said you can stay or leave. i said, i'm leaving. so waited half an hour, no one came to unhook my saline IV. i removed IV myself and left.
two hours later, in so much pain, i went 30 miles to a more rural and smaller hospital. told them i was in terrible pain, i had gone to the other hospital, had tests run and told them to get all the tests from them. within an hour the second hospital read the same tests the first hospital took and low and behold, i had pancreatitis and diverticulitis.
stayed in the hospital for three days on saline and morphine drip, no surgery.
