Smart has been used to refer to the causing of pain since Old English (our smart comes from the Old English smeart, meaning "causing pain"). This sense is still very much in use today, at least as a verb. Ouch, ouch, ouch—that smarts! —Rob Bacon, Logansport Pharos-Tribune (Logansport, IN), 26 Jul. 1961.
